CASE MANAGER

Prospect, one of Alberta’s most innovative and leading non-profits, is seeking a skilled Case Manager to join our team. You will have experience working with people with a broad range of disabilities including mental illness and developmental disabilities. Your approach to working with stakeholders and partners is highly innovative, creative, dynamic, collaborative, and outcomes focused.
Reporting to the Manager of Disability Services, this direct service position is responsible for coordinating all services a client experiences at Prospect in an efficient and organized manner.

PRIMARY RESPONSIBILITIES:
•Supporting the client intake process by: providing orientation to Prospect and program services, assessing individual client goals and needs, scheduling and coordinating intake meetings with clients and all members of their support network, arranging for accommodations or assistive technologies where appropriate, Scheduling annual review meetings, making decisions surrounding the acceptance of clients into service, developing employability assessments, service plans, employment profiles and client schedules.
• Act as the central point of contact for the client support network from intake to completion of service
•Monitor goal attainment including soliciting feedback from other departments such as job coaches and facilitators
•Maintains knowledge of supports and resources within the community including financial supports, funders, mental health service providers, supports for persons with developmental disabilities, crisis supports, behavioral supports, counseling supports and education and training supports.
• Other duties as assigned by supervisor

QUALIFICATIONS:
• Degree or certificate in related field from a recognized post secondary institution and 2 to 3 years experience in a similar role (equivalent combination of education and experience may be considered)
• Exceptional oral and written communication skills
• Demonstrated organizational and analytical ability
• Demonstrated proficiency with Microsoft Office products
• Previous experience with public speaking would be considered an asset
